Quality of life and standard of living in a randomly selected group of psychiatrically disabled people in Sweden 2 years after a psychiatry reform

In Sweden, a psychiatry reform, aimed at improving the living conditions of the psychiatrically disabled, came into force in 1995. The aim of the present study was to evaluate the impact of the reform by investigating quality of life and standard of living 2 years later in a randomly selected group of people with longstanding psychiatric disability. Self-ratings and interviews were conducted in a study group and a control group. The study group consisted of 19 women and 18 men (mean age 46.1 years) diagnosed with neurosis, schizophrenia or affective disorder. The control group consisted of 19 women and 17 men (mean age 48.7 years). Self-rated quality of life was significantly poorer in the study group (P < 0.0001, unpaired t-test), and so was housing (P < 0.001, test of similar proportions in independent samples). We found no significant positive correlation between subjective quality of life and standard of living in either group but a significant negative correlation in the control group (P < 0.05; r = 0.40, Pearson correlation coefficient). The results suggest that, in 1997, people with longstanding psychiatric disability still had poorer quality of life than the general population. This may be due to factors other than outward standard of living.

The concept of dementia should not be used synonymously with the concept of organic mental disorders. By definition, according to DSM-III and ICD-10, dementia is a syndrome which includes memory impairment. The severity of the disorder is disabling and the course is chronic. Differential diagnosis includes age-associated memory impairment (AAMI), delirium, and depressive disorders. The dementias may be subdivided into four groups: idiopathic (primary degenerative dementias), vascular, secondary, and others. The idiopathic dementias are those in which etiology is assumed to be found within the brain itself. The main subgroup is Alzheimer-type dementia. The vascular dementias are those in which the blood supply to the brain is insufficient. Multi-infarct dementia (MID) is the prototype. In secondary dementias, somatic disorders either within or external to the brain cause the dementia.

Patients (n = 191) living in four comparable somaic nursing homes (NH) (nursing homes for physical illness) were studied in order to evaluate dementia syndromes. Dementia and symptoms of depressed mood occurred frequently (72% and 63%, respectively). Dementia was often undiagnosed at admittance. Neither the length of time spent in institutions, nor marital status, age, or sex seemed to be of more than minor importance to the prevalence of dementia syndromes. Concerning functional impairment, convergence of findings across the societies studied indicates that psychiatric symptoms and psychopathology are intrinsic parts of long-term care of the elderly.

En Suecia, una reforma psiquiátrica dirigida a mejorar las condiciones de vida de los pacientes psiquiátricos discapacitados entró en vigor en 1995. El propósito del presente estudio era evaluar el impacto de la reforma investigando la calidad de vida y el nivel de vida 2 años después en un grupo seleccionado aleatoriamente de personas con discapacidad psiquiátrica de larga duración. Se realizaron autoevaluaciones y entrevistas en un grupo de estudio y uno de control. El grupo de estudio constaba de 19 mujeres y 18 hombres (edad media: 46,1 años) diagnosticados con neurosis, esquizofrenia o trastorno afectivo. El grupo de control constaba de 19 mujeres y 17 hombres (edad media: 48,7 años). La autoevaluación de la calidad de vida fue significativamente peor en el grupo de estudio (P < 0,0001, prueba de la t para datos independientes) y también lo fue la vivienda (P < 0,001, prueba de proporciones similares en muestras independientes). No encontramos correlación positiva significativa entre la calidad subjetiva de vida y el nivel de vida en ninguno de los dos grupos, pero hubo una correlación negativa significativa en el grupo de control (P < 0,05, r = 0,40, coeficiente de correlación de Pearson). Los resultados indican que, en 1997, las personas con discapacidad psiquiátrica de larga duración tenían todavía una calidad de vida peor que la población general. Esto se puede deber a otros factores que el nivel externo de vida.
Confusional State Evaluation (CSE): An instrument for measuring severity of delirium in the elderly

Background
Delirium or confusional state is a common mental disorder in the elderly. To follow changes in symptoms over time and to evaluate the efficacy of treatment of delirium, a reliable and valid instrument for measuring degrees of delirium is essential.
MethodAn observer's rating scale with 22 items, the Confusional State Evaluation (CSE), was developed. Scores on 12 of the items were summarised to a “confusion score”. Based on ratings of 7l demented and non-demented elderly patients with delirium, the interrater reliability and validity of the scale was studied.
ResultsAgreement between two independent raters was fair to excellent (weighted kappa 0.38–0.93). The correlation between the “consfucsoion score” of the scale and the global rating by a psychogeriatrician was good (r=0.79).
ConclusionsThe CSE seems to be a reliable and valid measuring instrument which can be useful in following the course of confusion in elderly patients.
Dexamethasone Suppression Test and Serum Prolactin in Dementia Disorders

The dexamethasone suppression test (DST) was performed on 21 patients with dementia of the Alzheimer type (DAT), 11 patients with multiinfarct dementia (Ml) and 14 healthy controls. Twelve of the DAT patients and eight of the Ml patients showed abnormal lack of suppression, compared with just one member of the control group. Abnormal DST was related to dementia as such and not to age or depression, or to levels of CSF monoamine metabolites. Basal serum prolactin concentrations were not increased.

Brain monoamine concentrations were determined post mortem in 19 patients with dementia of Alzheimer type. Samples were taken from 10 parts of the brain and compared with an age-matched control group.
There were lower mean concentrations of dopamine in the demented group of patients in seven regions of the brain, and two of these were at a significant level. There were also significantly lower concentrations of homovanillic acid in the nucleus caudatus and in the putamen. The means of the concentrations of noradrenaline were also lower, and in the putamen and the cortex gyrus frontalis significant differences were observed. The 5-hydroxytryptamine concentrations were slightly lower in the demented group but the differences did not reach significance. The degree of intellectual deterioration was negatively correlated with the noradrenaline concentrations in the hypothalamus and the cortex gyrus cinguli.

Determinations of acid monoamine metabolites, such as homovanillic acid (HVA) and 5-hydroxyindoleacetic acid (5-HIAA), in cerebrospinal fluid (CSF) give valid information on the metabolism of the corresponding amines in the brain tissue (Moir et al., 1970; Roos, 1970). The monoamine metabolites in the CSF are related to age. The concentrations of HVA and 5-HIAA increase with age (Gottfries et al., 1971). Probenecid blocks the elimination of HVA and 5-HIAA from brain tissue to blood (Neff et al., 1964, 1967; Werdinius, 1966) and from CSF to blood (Guldberg et al., 1966; Olsson and Roos, 1968). Probenecid thus normally induces an increase in the concentrations of the acid monoamine metabolites in the CSF, which is related to the turnover of monoamines in the brain tissue.

Flupenthixol is the latest addition to the thioxanthene series (Møller Nielsen, 1967). Clinical investigations have shown that it is principally indicated in conditions of prolonged psychosis without severe anxiety (Gross and Kaltenbäck, 1965; Knutsen, 1965, Noreik and Rimestad, 1965; Gottfries, 1967). An antidepressant effect and an effect on neurosis have also been reported, (Ravn and Rud, 1965; Sonne, 1966), and there have been tests with the preparation as a depot-neuroleptic (Enerheim et al., 1970).

In a previous investigation, the concentration of homovanillic acid (HVA) seemed to be related to the degree of dementia (Gottfries et al., 1965). The object of the present work is the further study of the occurrence of HVA in the human brain, especially with regard to senile dementia. The hypothesis is that senile dementia is a disease with a disturbance in the monoamine metabolism, which is reflected in a reduction of HVA in the neostriatum.
